RESUMO

A myxoid cyst on the dorsum of the left index finger is presented. A connection of this pseudocyst to the underlying joint was shown by means of magnetic resonance imaging. Surgically the connecting tract to the distal interphalangeal joint was easily demonstrated and histologically a ductal structure was focally seen by means of serial sections. These structures also suggest such a connection. These findings confirm the view that this pseudocyst can be interpreted as a ganglion. Therapeutically complete excision with careful tying off of the channel to the joint appears to be the best method to avoid recurrence. The injection of sclerosing agents, a conservative treatment modality proposed by some authors, may be problematic in the light of the pathogenesis discussed here; however damage to the finger joint has not been so far observed.

RESUMO

The coverage of extensive defects of the scalp is sometimes a problem. In some clinical cases the methods of definitive coverage are shown. Microsurgery permits the replantation of a scalp or the coverage of a defect with a free flap. A new technique consists in the use of a tissue expander to stretch the remaining scalp.

RESUMO

One hundred late results of augmentation mammaplasty for the following conditions are reported: aplasia, hypoplasia, asymmetry and deformities after total and partial mastectomy. Our criteria for assessment were shape, consistency and quality of scars. The objective assessments were compared with the patients subjective assessments. The most unsatisfactory results were in augmentation after subcutaneous mastectomy and the best were in hypoplasias when small implants were used. Two types of implant were compared: gel filled and inflatable. Results were slightly better with gel filled implants. Better results were obtained if the implant was placed subpectorally and postoperative massage was carried out. Special note is made of the high rate of fibrous capsule contraction.

RESUMO

Of 1732 patients, 95% were followed up. The proportion of men to women was 7:1, most patients being about 70 years old. Eighty percent of the patients smoked, 64% were considered inoperable when first seen, and 4% were found to be incurable at the time of surgery. Postoperative deaths were 8.2%; 3 bronchopleural fistulas and 17 pleural empyemas complicated recovery. Of those patients operated on without lymph node involvement, the 5-year survival rate was 45.4%, with lymph node metastasis 14.7%. Of 242 patients with oat cell carcinoma who were not operated on, 85% died within the first year after hospital discharge. Only 1.2% survived 5 years.
